How to refer

Send the participant’s details.

Email info@circlewell.com.au with the plan reference, management type (self / plan / agency [VERIFY]), the disciplines needed, and the participant’s goals if you have them.

We confirm within two business days.

Availability per discipline, our rate (at or under the NDIS price cap), the first appointment window, and the timeline for the first report. Sample reports available on request.

Service agreement, then first session.

Rate and cancellation terms confirmed in writing before the first appointment. The clinician who starts stays for the length of the goal — and writes the plan-review report.

What you’ll receive back

Rate transparencyAt or under the NDIS cap, in the service agreement, before the first session. No surprise line items, no case-conferencing hours nobody consented to.
Reports included in session hoursPlan-review reports written in plain words the participant, you, and the NDIA can all read — no separate billing.
Continuity you can promise participantsNo clinician roster rotation. The person who starts finishes.
Honesty about capacityIf our waitlist doesn’t serve the participant’s timeline, we say so up front and help you find an alternative.
